Gero Community Stabilization Program

The Gero Community Stabilization Team (GCST) is an interdisciplinary team that helps support adults aged 65 or older with dementia, Alzheimer’s, or a serious mental illness in jeopardy of being hospitalized, incarcerated, or homeless due to behaviors who live in areas served by Region Five. This program makes it possible to discharge older adults from the hospital to their home or nursing homes and assisted living facilities by serving as a step-down service that provides crisis stabilization and intensive mental health services.

GCST services include behavior management and interventions including but not limited to:

  • Structured activities and linkage to other services and supports
  • Training for the individual’s system of support to include family and providers
  • Consultations with providers in Assisted Living Facilities and Nursing Homes.

Who can access this service?

We support residents of Region Five that are 65 years of age or older. All participants must be diagnosed with a qualifying mental health disorder of dementia or Alzheimer’s with behaviors and be at risk of hospitalization or loss of placement.
