Gero Community Stabilization Program

The Gero Community Stabilization Team (GCST)provides mental health support to help older adults stay independent and stable in the community and avoid unnecessary psychiatric hospitalizations. GCST serves individuals living in the Region Five area who are diagnosed with a qualifying mental health disorder, including dementia or Alzheimer’s with behaviors, and who are at risk of hospitalization or loss of placement.

GCST services include behavior management and interventions, including but not limited to:

  • Structured activities and linkage to other services and supports
  • Training for the individual’s system of support, including family and providers
  • Consultations with providers in Assisted Living Facilities and Nursing Homes

GCST also offers regional trainings to help increase understanding of dementia and neurocognitive disorders for individuals, caregivers, community members, and professionals. These psychoeducational trainings support symptom management and general well-being. Please contact us if you are interested in scheduling a training.

Program Eligibility:

  • Be a resident of the Region Five service area
  • Be sixty-five (65) years of age or older
  • Be diagnosed with a qualifying mental health disorder, including dementia or Alzheimer’s with behaviors
  • Be at risk of hospitalization or loss of placement
